The gastric mucosa exhibits a polypoid architecture with focal cystic dilatation of fundic glands and reactive foveolar hyperplasia. The surface demonstrates extensive erosions, fibrinopurulent exudate, and prominent hemorrhage. The lamina propria is expanded by a dense mixed inflammatory infiltrate showing neutrophilic infiltration of the epithelium. Helicobacter pylori are identified within the surface mucus and gastric pits. No intestinal metaplasia or dysplasia is identified.
Severe erosive and hemorrhagic gastritis with diffuse, predominantly fundic, gastric polyps.
