<ul><li class="ql-align-justify">Ileal mucosa showing focal superficial erosion with moderate infiltration by neutrophils, lymphocytes and plasma cells. Villi are intact and are negative for atrophy. No evidence of specific granulomas. No evidence of malignancy.</li><li class="ql-align-justify">Polypoid lesions formed of hyperplastic intact colonic glands with preserved mucigenic activity. Stroma is fibrotic and shows mild chronic inflammatory reaction composed of lymphocytes and plasma cells. No evidence of specific granulomas. No evidence of crypt abscesses. No evidence of atypia or malignancy.</li></ul>
<p class="ql-align-justify">Fissuring nodular mucosa in the terminal ileum—suspicious for inflammatory bowel disease (IBD)—alongside two small polyps in the ascending colon that were removed by forceps.</p>
<p><strong>Two biopsies were received:</strong></p><ul><li class="ql-align-justify"><strong>Biopsy A (Labeled "Terminal Ileum"):</strong> fragments of soft tissue, collectively measuring 0.8 cm in greatest dimension, totally embedded.</li><li class="ql-align-justify"><strong>Biopsy B (Labeled "Polyp"):</strong> fragments of soft tissue, collectively measuring 0.7 cm in greatest dimension, totally embedded.</li></ul>
<h3><strong><em><u>Terminal ileum, endoscopic biopsy:</u></em></strong></h3><ul><li><strong>Moderate chronic non-specific inflammation with superficial erosion and inflammatory activity. </strong></li><li><strong>Negative for granulomas. </strong></li><li><strong>Negative for malignancy. </strong></li></ul><p><br></p><p><strong><em><u>Ascending Colon polyps, endoscopic polypectomy:</u></em></strong></p><ul><li><strong>Hyperplastic polyps, inflamed. </strong></li><li><strong>Negative for malignancy</strong></li></ul>
