Appendicular lumen is distended and appendicular epithelium is extensively ulcerated and reduced in few foci to a flat layer of simple columnar mucigenic epithelium. Foci of ulceration are seen related to dense non-specific inflammation rich in neutrophils, lymphocytes, plasma cells with histiocytes. This infiltrate extends to involve the whole thickness of the thick and fibrotic wall and into periappendiceal fat with perforation. No evidence of malignancy.

The specimen consists of an appendix measuring 7x1, with attached small fat measuring 4x1. It shows a cyst measuring 5.8x1 that is filled with brownish material.
