Tissue sections show a cellular astroglial tumor formed of pleomorphic and dominantly multinucleated cells with separating fibrillary stroma. Tumor cells are seen arranged radially around foci of tumor necrosis.Tumor is vascular and there is focal evidence of endothelial proliferation with glomeruloid bodies formation. Cytology preparations show scattered atypical cells with irregular and hyperchromatic nuclei, arranged singly and in cohesive clusters. This is a WHO grade IV tumor.
The patient presents with a ring-enhancing left frontal brain mass that intraoperatively appeared as a cystic lesion with surrounding yellowish-red edema and greenish-yellow, cheesy, caseation-like tissue.
Two samples were received:
1- Tissue biopsy: soft tissue fragments collectively measured 5.5x4.5x0.8 cm, totally embedded.
2- Aspiration cytology: One smear was prepared and stained from submitted 1 ml reddish fluid.
