Sections examined from the distal segment show absence of ganglion cells of the intramuscular and submucosal plexus (Meissner's plexus) with hypertrophied nerves. The proximal and distal segments show normally ganglionated submucosal and intramuscular nerve plexuses (Meissner's and Auerbach's nerve). Dissected five regional lymph nodes are reactive (0/17). No evidence of specific granulomas. No evidence of atypia or malignancy.
Hirschsprung's disease.
Three specimens were submitted:
(1) Proctosigmoidectomy specimen; Colonic segment 30 cm long with attached omental tissue measures 9x5 cm; the distal part is narrow measuring 15 cm long and 4 cm in diameter followed by gradually dilated segment with proximal diameter 6 cm. Sections were submitted along the whole length (1 to3 : Proximal to distal). No evidence of mucosal or mural tumors. Dissected lymph nodes are soft 0.2-0.5 cm
(2) Proximal margin: Colonic fragment 6x2x1.5 cm, grossly unremarkable.
(3) Distal margin: Colonic fragment 2x1.7x1.5 cm, grossly unremarkable.
