Diagnosis
<p>(1) Endometrium is non-secretory and hyperplastic, it is formed of branching glands exhibiting focal moderate crowdness and lined by 1-3 layers of cells. Glands show focal complexity of their contour. Stroma is cellular. Islands of these endometrial glands and stroma are seen entrapped deep within myometrium. Fibrotic cyst wall showing partial lining by polygonal inner theca and outer granulosa cells with mild luteinization of these cells. Cervix shows Nabothian cysts. No evidence of atypia or malignancy.</p><p>(2) Smear is hemorrhagic and show a very mild number of small lymphocytes with few reactive mesothelial cells. No evidence of atypical or malignant cells.</p>
<p>History of vaginal bleeding and right ovarian multilocular cyst.</p>
<p>Two specimens were submitted: (1) Total hysterectomy with unilateral salpingo-oophorectomy specimen; Uterus 9x7x4.5 cm with 2.2 cm thick wall. Cervix is 3 cm long. Rt. Ovary measures 4.2x3x1.5 cm, cut section is cystic, contains clear fluid with intact capsule. (2) Peritoneal wash, cytology: One smear was made from submitted 10 ml reddish fluid and stained.</p>
<p><strong><em><u>(1) Total hysterectomy with unilateral salpingo-oophorectomy:</u></em></strong></p><ul><li><strong>Moderate adenomatous (complex) endometrial hyperplasia. </strong></li><li><strong>Deep adenomyosis.</strong></li><li><strong>Ovarian solitary follicular cyst. </strong></li><li><strong>Cervical Nabothian cysts.</strong></li><li><strong>Negative for atypia. </strong></li></ul><p><br></p><p><strong><em><u> (2) Peritoneal wash, cytology:</u></em></strong></p><ul><li><strong>Hemorrhagic and hypocellular material.</strong></li><li><strong>Negative for malignancy. </strong></li></ul>
