Diagnosis
<p class="ql-align-justify">Thyroid gland shows nodular adenomatous hyperplasia. Nodules are variable in size and composed of small and large crowded follicles full of thick colloid and lined by mature epithelium. Sections from some of the nodules show compact microfollicles devoid of colloid and lined by cuboidal cells separated by delicate vascular stroma. In these follicles nuclei are vesicular and look active but no appreciable mitosis. Stroma shows mild fibrosis and show focal dense infiltration by lymphocytes amounting at the formation of lymphoid follicles with intact active germinal centers. Related follicles show degeneration but no evidence of oxyphil change of the follicular epithelium. Foci of calcification are noted. No evidence of hyperfunction or malignancy.</p>
<p class="ql-align-justify">Neck swelling. U/S: both thyroid nodules and isthmus are grossly enlarged in size and heterogeneous echopattern with multiple variable size 2-3 cm iso hypoechoic partially defined mostly solid nodules.</p>
<p class="ql-align-justify">Total thyroidectomy specimen showing two lobes measure 10 x 5.5 x 4.5 cm and 9 x 5.5x 4.5 cm with isthmus 1.5 x 1 cm. Cut sections showing multiple soft nodules the largest measured 2.3 x 2 cm and the small measured 0.6 cm, with calcifications in areas of 1.3 x1 cm and 0.6 cm. Thyroid capsule is intact.</p>
<p class="ql-align-justify"><strong><em><u>Thyroid gland, total thyroidectomy: </u></em></strong></p><ul><li class="ql-align-justify"><strong>Nodular adenomatous hyperplasia. .</strong></li><li class="ql-align-justify"><strong>Associated lymphocytic thyroiditis. </strong></li><li class="ql-align-justify"><strong>Negative for malignancy</strong>.</li></ul>
