Microscopic examination of the sigmoid colon reveals multiple herniations of the mucosa and submucosa extending through the muscularis propria, accompanied by prominent hypertrophy of the muscularis layer, characteristic of diverticulosis. The adjacent colonic mucosa is unremarkable and shows no architectural distortion. Sections of the appendix demonstrate a variably thickened wall characterized by mural fibrosis and a chronic lymphoplasmacytic inflammatory infiltrate expanding the lamina propria and submucosa, consistent with chronic appendicitis.
The specimen consisted of a segment of sigmoid colon measuring 21 cm long with attached mesenteric fat measuring 11 x 3 cm. Opening and careful examination did not reveal any gross masses or ulcerations. Received separately was an appendix measuring 5.5 x 0.5 cm with attached fat measuring 5 x 2.5 cm. The appendix exhibited ulcerated mucosa with thickened wall measuring up to 0.4 cm.
