Diagnosis
<p class="ql-align-justify">Sections show caseating granulomas rich in epithelioid cells and occasional Langhans' giant cells. Granulomas show evidence of focal secondary infection manifested by focal mild infiltration by small lymphocytes, plasma cells and neutrophils. Smear shows a moderate number of neutrophils and small lymphocytes with scattered histocytes. Picture is consistent with a tuberculous etiology. No evidence of malignancy.</p>
<p class="ql-align-justify">Multiple discrete and confluent cystic lesions in the bilateral cerebral hemispheres and basal ganglia.</p>
<p class="ql-align-justify">Soft tissue piece measuring 3 x 2.3 x 1.8 cm, containing a cyst measuring 2.2 x 2 cm with a 3 mL of pale yellow fluid, from which one smear was prepared and stained.</p>
<p><strong><em><u>Brain cystic lesion, biopsy and aspiration cytology:</u></em></strong></p><ul><li><strong>Secondary infected caseating granulomatous </strong></li><li><strong>Suggestive of tuberculous etiology.</strong></li><li><strong>Negative for malignancy. </strong></li></ul><p><br></p><p><strong><u>COMMENT:</u></strong></p><h5>Confirmation of this etiology by microbiological studies and skin testing is recommended. </h5>
