Diagnosis
<p class="ql-align-justify">Focally ulcerated colonic mucosa mildly infiltrated by lymphocytes and plasma cells with lymphoid follicles. Mucigenic cells are intact and no evidence of crypt abscesses. No evidence of atypia or malignancy.</p>
<p class="ql-align-justify">Blleeding per rectum was found on colonoscopy to have localized hyperemia and a few tiny erosive spots in the distal rectum.</p>
<p class="ql-align-justify">Soft tissue fragments collectively measured 0.9 cm, totally embedded.</p>
<p><strong><em><u>Rectum, endoscopic biopsy: </u></em></strong></p><ul><li><strong>Mild chronic follicular colitis with focal superficial erosions. </strong></li><li><strong>Negative for dysplasia and malignancy. </strong></li></ul>
