Ileal mucosa shows focal ulceration with related dense infiltration by lymphocytes and plasma cells and perforation. Related serosa shows moderate to marked infiltration by inflammatory cell with fibrosis. No evidence of specific granulomas.
Following an IUD procedure, the patient developed peritonitis with abdominal pain, necessitating an exploratory laparotomy that revealed a distal ileum perforation, the margins of which were sent for histopathology to rule out inflammatory bowel disease.
Small bowel segment designated as "distal ileum," measuring 2.8 x 1 x 1 cm, totally embedded.
