Sections show a smooth muscle neoplasm composed of intersecting fascicles of spindle-shaped cells with eosinophilic cytoplasm and elongated, blunt-ended nuclei. The tumor exhibits variable cellularity with focal areas of mild to moderate nuclear atypia, characterized by nuclear enlargement, irregular nuclear contours, and occasional hyperchromasia. Mitotic activity is low to intermediate and does not reach the threshold for leiomyosarcoma, with mitotic figures present but not atypical. Coagulative tumor cell necrosis is absent. The tumor margins are generally well circumscribed, without unequivocal infiltrative growth into the surrounding myometrium. Overall, the constellation of histomorphologic features does not fulfill established criteria for benign leiomyoma or leiomyosarcoma, supporting a diagnosis within the spectrum of smooth muscle tumor of uncertain malignant potential.
