Endometrium shows hyperplastic endometrial glands, is non-secretory, it is formed of slightly branched glands lined by 1-2 layers of cells. Stroma is cellular. No evidence of complex glandular pattern. Sections from the polypoid mass revealed polypoid lesion composed of irregular, branching and focally endometrial glands lined by 1-3 layers of non-secretory columnar cells. Separating stroma is abundant and contains bundles of smooth muscles. No evidence of atypia or malignancy. Cervix show mild endocervicitis. Both ovaries show corpora albicans and cystic follicles. Fallopian tubes are unremarkable.
Abnormal vaginal bleeding for 3 years: bulky uterus featuring a large, enhancing, pedunculated submucosal uterine fibroid that extends through the cervical canal and protrudes into the posterior vagina, accompanied by a small intramural fibroid.
Panhysterectomy: uterus measured 11 x 7.5 x 6 cm, cervix measuring 3 cm in length. On sectioning, the myometrial wall thickness measured 3 cm. The endometrial cavity was almost entirely occupied by a gray-white polypoid mass measuring 8 x 4.5 x 4 cm. Rt. and Lt. ovaries measured 2 x 1.3 x 1 cm each. Rt. and Lt. fallopian tubes measured 5 cm, each.
