Microscopic Description
Diagnosis
<p>Liver parenchyma with marked infiltration of portal areas by mature lymphocytes which appear to extend in many microscopic foci to encircle degenerating hepatocytes at periphery of lobules (interface hepatitis/ piece-meal necrosis). Portal areas show moderate fibrosis with evidence of bridging fibrosis (porto-portal & zone III-portal) but no evidence of frank cirrhosis. Many bile ducts showing bile pigments denoting cholestasis. Associated moderate fatty change. No evidence of specific granulomas. No evidence of dysplasia or malignancy. </p>
<p>Guided core biopsy from hepatic focal lesions. ? Metastasis or Lymphoma. </p>
<p>Submitted one paraffin block and 2 Hx & E stained slides, coded 2155-26</p>
<p><strong><em><u>Hepatic focal lesion, guided core biopsy, referred paraffin block and slides for review:</u></em></strong></p><ul><li><strong>Grade 3 chronic hepatitis with associated stage 3 fibrosis (METAVIR A2F3, Ishak: grade 7-8/18, stage 4-5/6). </strong></li><li><strong>Associated cholestasis and moderate fatty change. </strong></li><li><strong>Negative for granuloma, dysplasia and malignancy. </strong></li></ul><h5><br></h5><h5><strong><u>COMMENT:</u></strong></h5><h5>Given clinical findings regrading presence of multiple lesions, please make sure the submitted material is representative. Otherwise rebiopsy will be recommended if malignancy is still suspicious. </h5><p><br></p>
