Submitted material is dominantly formed of unremarkable lamellar bone and cartilaginous tissue separated by normocellular bone marrow for age. Few foci showing focal involvement of bone marrow by atypical round cells exhibiting eccentric nuclei, coarse chromatin, and moderate amounts of amphophilic cytoplasm. Picture is suspicious of malignancy, possibly plasma cell myeloma.
Aggressive bone lesion involving the D5 and D6 vertebrae with an associated soft tissue component causing spinal canal stenosis and suspected cord infiltration.

COMMENT:
Due to scarcity of the atypical cells, immunostaining could be of no value. However, a trial at marker study (CD138, MUM1, CD20, CK, Synaptophysin) may be attempted before considering more abundant rebiopsy.
