Sections show architectural distortion with dense collagenous fibrosis and cystic spaces lined by respiratory epithelium, characteristic of honeycombing. The parenchyma shows patchy chronic inflammation and scattered fibroblastic foci. Three identified lymph nodes exhibit reactive hyperplasia and heavy anthracotic pigment deposition. No granulomas or malignancy are observed.
15-year history of chronic chest infections diagnosed as chronic bronchiectasis and fibrosis of the left lung, who has now undergone a total left pneumonectomy.
The specimen consisted of a left lung lobe measuring 9.5 x 7 x 4.5 cm and weighing 38 grams. The pleural surface appeared thickened with focal adhesions. Upon sectioning, the parenchyma showed diffuse, white, firm fibrosis accompanied by subpleural honeycombing. No masses or lymph nodes were identified during the macroscopic examination.
