Diagnosis
<ul><li class="ql-align-justify">Intact colonic mucosa moderately infiltrated by lymphocytes and plasma cells with lymphoid follicles. Mucigenic cells are mildly decresead. Some neutrophils are seen infiltrating glands but there is no evidence of crypt abscesses. No evidence of atypia or malignancy.</li><li class="ql-align-justify">Polypoid colonic lesions composed of hyperplastic tubular glands with minor villous configuration. Lining columnar cells show focal mild dysplasia but no evidence of malignancy. Complete excision of this polyp is recommended. </li></ul>
<p>Hyperemic mucosa in the sigmoid colon and two sessile polyps.</p>
<p class="ql-align-justify">Two biopsies were received:</p><p class="ql-align-justify">1- Sigmoid: soft tissue fragments collectively measured 0.6 cm, totally embedded.</p><p class="ql-align-justify">2- Two small sessile polyps removed by forceps: soft tissue fragments collectively measured 0.6 cm, totally embedded.</p>
<p><strong><em><u>Colon, endoscopic biopsy and polypectomy:</u></em></strong></p><ul><li><strong>Moderate chronic follicular colitis with mild inflammatory activity. </strong></li><li><strong>Negative for dysplasia and malignancy. </strong></li><li><strong>Tubular adenomatous polyp with low grade dysplasia</strong></li><li><strong>Negative for atypia and malignancy.</strong></li></ul>
