<p>• <strong>Procedure: </strong>RIGHT MODIFIED RADICAL MASTECTOMY:</p><p>•<strong> Histologic diagnosis and grade: </strong>Multicentric invasive carcinoma of no special type, NST (= Invasive duct carcinoma), grade 3.</p><p>• <strong>Tumor size:</strong> largest invasive focus measures 3.5 cm.</p><p>• <strong>Associated DCIS:</strong> present, minor component (~10%).</p><p><strong>• Lymphovascular invasion:</strong> present.</p><p><strong>*Margins:</strong> Negative for tumor extension.</p><p>• <strong>Axillary lymph nodes:</strong> 26/27 positive for metastatic carcinoma.</p><p>• <strong>Extranodal extension: </strong>present.</p><p>•<strong> PATHOLOGIC STAGING:</strong></p><p>• pT2 pN3aMx.</p><p><br></p><p><br></p><p><br></p><p><br></p><p><br></p><p><br></p><p><br></p>
<p class="ql-align-justify">Lactating female presenting with a huge, locally advanced right breast mass clinically suspicious for inflammatory breast cancer (BIRADS V), characterized by diffuse edema, skin thickening with a "peau d'orange" appearance, and multiple matted axillary and supraclavicular lymphadenopathy.</p>
<p>Two specimens were received:</p><p class="ql-align-justify">Specimen 1 consists of a Modified Radical Mastectomy (MRM) of the right breast measuring 29 × 19 × 8 cm, covered by a skin ellipse measuring 27 × 18 cm with unremarkable nipple and areola. Attached is an axillary tail measuring 7 × 5.5 × 3 cm.</p><p class="ql-align-justify">Sectioning revealed three solid, firm, cream-colored masses. The largest mass measures 3.5 × 2.5 × 2 cm, with distances inbetwen 3 cm, 5 cm and 6 cm. Least deep margin is 0.5 cm. A fibrotic gray area measuring 5 × 4 cm is also identified. Surgical margins are as follows: superior 4 cm, inferior 12 cm, medial 11 cm, and lateral 7 cm.</p><p class="ql-align-justify">Dissection of the axillary tail revealed 21 lymph nodes, the largest measuring 4.5 cm.</p><p class="ql-align-justify">Specimen 2 consists of five pieces of fatty soft tissue, collectively measured 9 × 7 × 5.5 cm. Twenty-one lymph nodes are identified, the largest measuring 4.5 cm. Dissection revealed 8 lymph nodes, largest 1.7 cm.</p>
