• Procedure: RIGHT MODIFIED RADICAL MASTECTOMY:
• Histologic diagnosis and grade: Multicentric invasive carcinoma of no special type, NST (= Invasive duct carcinoma), grade 3.
• Tumor size: largest invasive focus measures 3.5 cm.
• Associated DCIS: present, minor component (~10%).
• Lymphovascular invasion: present.
*Margins: Negative for tumor extension.
• Axillary lymph nodes: 26/27 positive for metastatic carcinoma.
• Extranodal extension: present.
• PATHOLOGIC STAGING:
• pT2 pN3aMx.
Lactating female presenting with a huge, locally advanced right breast mass clinically suspicious for inflammatory breast cancer (BIRADS V), characterized by diffuse edema, skin thickening with a "peau d'orange" appearance, and multiple matted axillary and supraclavicular lymphadenopathy.
Two specimens were received:
Specimen 1 consists of a Modified Radical Mastectomy (MRM) of the right breast measuring 29 × 19 × 8 cm, covered by a skin ellipse measuring 27 × 18 cm with unremarkable nipple and areola. Attached is an axillary tail measuring 7 × 5.5 × 3 cm.
Sectioning revealed three solid, firm, cream-colored masses. The largest mass measures 3.5 × 2.5 × 2 cm, with distances inbetwen 3 cm, 5 cm and 6 cm. Least deep margin is 0.5 cm. A fibrotic gray area measuring 5 × 4 cm is also identified. Surgical margins are as follows: superior 4 cm, inferior 12 cm, medial 11 cm, and lateral 7 cm.
Dissection of the axillary tail revealed 21 lymph nodes, the largest measuring 4.5 cm.
Specimen 2 consists of five pieces of fatty soft tissue, collectively measured 9 × 7 × 5.5 cm. Twenty-one lymph nodes are identified, the largest measuring 4.5 cm. Dissection revealed 8 lymph nodes, largest 1.7 cm.
Right breast, modified radical mastectomy:
