Four fragments show partially ulcerated gastric mucosa with related malignant tumor composed of large rounded epithelial cells having irregular hyperchromatic nuclei and focally abundant eosinophilic cytoplasm. Anaplasia and mitosis are marked. Picture is consistent with undifferentiated carcinoma, large cell type. In view of tumor site (gastro-esophageal junction) and cytomorphology, it is advised to exclude the possibility of poorly differentiated squamous cell carcinoma extending from cardia before considering the possibility of poorly differentiated adenocarcinoma. Careful correlation with endoscopic and imaging findings and marker study (p63, CEA and CDX2) may be of help in this respect. Rest of fragments show H pylori chronic active gastritis.
