Diagnosis
<p class="ql-align-justify">Sections show fragments composed of proliferating fibroblastic stromal cells with plump nuclei admixed with evenly distributed osteoclast-type multinucleated giant cells. Foci of numerous blood vessels with associated interstitial hemorrhage are present. No cytologic atypia or malignant features are identified.</p>
<p class="ql-align-justify">Rt. patellar tumor, by MRI suspected giant cell tumor or bone cyst.</p>
<p class="ql-align-justify">Bloody soft tissue fragments collectively measured 4.5x4x0.8 cm, totally embedded.</p>
<p><strong><em><u>Rt. patellar tumor, biopsy:</u></em></strong></p><ul><li><strong>Giant cell–rich lesion of bone.</strong></li></ul><p><br></p><p><strong>Comment</strong></p><h5 class="ql-align-justify">The histomorphology raises the possibility of a brown tumor of hyperparathyroidism (osteitis fibrosa cystica / von Recklinghausen disease of bone). Correlation with clinical findings, skeletal survey, serum parathormone, calcium, and phosphorus levels, as well as radiological findings on X-ray and CT scan, is recommended. In the absence of evidence of hyperparathyroidism, the differential diagnosis includes giant cell tumor of bone, aneurysmal bone cyst, and chondroblastoma, with radiologic correlation being essential for further characterization.</h5>
