Pathologic Staging (AJCC 8th Edition)
Pyloric mass diagnosed as poorly differentiated a denocarcinoma, non signet ring type grade 3 (pathology number 2846-2026 on 2 May 2026).
Subtotal distal gastrectomy: The specimen consisted of a portion of the proximal duodenum and a distal stomach measuring 10.5 cm along the greater curvature, 8.5 cm along the lesser curvature, 6 cm in proximal circumference, and 3.5 cm in distal circumference. Attached was a greater omentum measuring 30 x 25 x 1.6 cm, accompanied by an additional attached piece of fatty tissue measuring 10 x 7 cm. Opening of the stomach revealed an ulcerated, solid, firm, creamy-colored mass measuring 1.8 cm in axial length and 1.1 cm in depth. The mass is grossly infiltrating submucosa. The mass was located 8.5 cm from the proximal resection margin and 1 cm from the distal resection margin. A total of 24 lymph nodes were dissected along the lesser and greater curvatures, with the largest lymph node measuring 1.7 cm in greatest dimension.
