Malignant epithelial tumor infiltrating mature adipose tissue in both biopsies with no evidence of lymphoid architecture. The tumor displays solid, nested, and prominent micropapillary growth patterns embedded within a dense desmoplastic stroma containing focal calcifications. The tumor cells exhibit severe nuclear pleomorphism, high nuclear-to-cytoplasmic ratios, and frequent mitotic activity. Immunohistochemical correlation using PAX8, WT1, CK7, SATB2 and CDX2 is recommended to confirm Müllerian lineage and definitively exclude a gastrointestinal primary.
