Diagnosis
<ul><li class="ql-align-justify">Core biopsy is dominantly composed of fibrotic tissue entrapping moderate inflammatory infiltrate formed mainly of lymphocytes and plasma cells with related capillary proliferation. Rest of core showing patent lung alveoli. No evidence of specific granulomas. No evidence of atypia or malignancy.</li><li class="ql-align-justify">Smear shows a dense population of neutrophils with some small lymphocytes as well as reactive mesothelial cells. No evidence of atypical or malignant cells.</li></ul>
<p class="ql-align-justify">Lung empyema, suspected TB versus malignancy.</p>
<p>Two samples were submitted:</p><p class="ql-align-justify">1- Coe biopsy: two tissue cores up to 1.3 cm, totally embedded.</p><p class="ql-align-justify">2- Pleural fluid US guided FNAC: 1 ml of bloody mucous with pus, one smear was prepared and pap stained.</p>
<p><strong><em><u>LUNG, CORE BIOPSY:</u></em></strong></p><ul><li><strong>Chronic non-specific inflammation and fibrosis.</strong></li><li><strong>Negative for specific granulomas. </strong></li><li><strong>Negative for malignancy.</strong></li></ul><p><br></p><p><strong><em><u>PLEURAL FLUID, CYTOLOGY:</u></em></strong></p><ul><li><strong>Marked (suppurative) inflammatory changes.</strong></li><li><strong>Negative for malignancy.</strong></li></ul>
