Submitted brain glial tissue shows dense infiltration by neutrophils, lymphocytes, plasma cells and macrophages with related capillary proliferation and cell debris. No evidence of granulomas or tumors in this material.
History of a brain abscess excised 9 years ago, presenting with a 5-day history of headache, whose current brain MRI reveals a large conglomerate of ring-enhancing lesions in the right temporo-parieto-frontal cortex causing significant mass effect, a midline shift to the left, and right uncal herniation—highly suspicious for recurrent or new cerebral abscesses—alongside stable left parieto-occipital encephalomalacia and a left-sided VP shunt.
Soft tissue fragments measuring in aggregate 4.5 x 3.8 x 1.3 cm; the largest measures 1.8 x 1.5 x 1.3 cm, totally embedded.
