Skin tissue overlying an infiltrative dermal malignant neoplasm arranged in nodular and lobular configurations. The tumor is composed of undifferentiated basaloid cells with hyperchromatic, elongated nuclei, high nuclear-to-cytoplasmic ratios, and prominent mitotic figures. Interspersed within these islands are clusters of neoplastic cells showing classic sebaceous differentiation, characterized by abundant, vacuolated, foamy cytoplasm and indented nuclei. Foci of central comedo-type necrosis and loose myxoid change are present. The surrounding fibrovascular stroma is well-vascularized, with focal peripheral palisading at the lobular margins. Immunostaining was done with appropriate positive and negative controls and revealed: diffuse cytoplasmic positivity for EMA and CK7, along with focal immunoreactivity for BCL2.
