Appendicular lumen is distended and appendicular epithelium is extensively ulcerated and reduced in few foci to a flat layer of simple columnar mucigenic epithelium. Foci of ulceration are seen related to dense non-specific inflammation rich in neutrophils, lymphocytes, plasma cells with histiocytes. This infiltrate extends to involve the whole thickness of the thick and fibrotic wall. No evidence of malignancy.

Appendicectomy specimen showing appendix 6x1 cm long with attached mesoappendix measuring 3 cm. Opening revealed ulcerated mucosa and almost complete replacement of appendix by a cyst filled with brownish fluid with, wall thickness up to 0.3 cm. Capsule is intact.
