Sections reveal gastric mucosa extensively infiltrated by dense chronic inflammatory cells, accompanied by moderate to marked neutrophilic activity and conspicuous eosinophilic infiltrates. Foveolar and glandular epithelial injury with superficial mucosal erosions are present. The gastric epithelium exhibits moderate reactive atypia/dysplastic change characterized by nuclear enlargement, hyperchromasia, stratification, and regenerative architectural distortion in a background of active inflammation. Helicobacter pylori organisms are identified adherent to the surface foveolar epithelium and within the overlying mucus layer. No evidence of frank malignancy is identified.

The epithelial atypia is interpreted in the context of severe active Helicobacter pylori gastritis and mucosal injury. Eradication therapy is recommended, followed by interval endoscopic biopsy sampling after treatment to document resolution of inflammation and reassess the epithelial atypia/dysplastic alterations.
