Endometrium is non-secretory, it is formed of slightly branching glands lined by 1-3 layers of cells. Stroma is cellular. No evidence of complex glandular pattern. Some of these hyperplastic glands are seen embedded within deep myometrium. Myometrial tumors are benign leiomyomas formed of whorly bundles of mature smooth muscles and separated by a fibrous stroma. No evidence of malignancy. Cervix shows Nabothian cysts with mild endocervicitis. Both ovaries show cystic follicles. Fallopian tubes are unremarkable. No evidence of atypia or malignancy.
Mass protruding from vagina, on examination: bulky cervix.
Panhysterectomy specimen: The uterus measured 10.5 x 7 x 5.5 cm. On sectioning, the myometrium measured 2.7 cm showing multiple fibroid masses, the largest measuring 1.5 x 1.3 cm. Endometrium measured 0.5 cm in thickness. The cervix measured 3 cm in length. The right and left ovaries measured 3 x 1.5 x 1.5 cm and 3.5 x 1.5 x 1.5 cm, both showing unilocular cyst measured 1.5 cm and 1 cm, respectively. The right and left fallopian tubes measured 5 cm and 5.5 cm, respectively, grossly unremarkable.
