Sections of the gallbladder show transmural chronic inflammation with extensive mucosal ulceration and marked wall thickening. Other areas show dense sheets of lipid-laden foamy histiocytes intermixed with lymphocytes, plasma cells, and neutrophils extending deeply through the muscularis into the adventitia. This inflammatory process is accompanied by prominent reactive fibroblastic proliferation and dense collagenous fibrosis. No epithelial dysplasia or malignancy is identified.
Recurrent calculous cholecystitis and intraoperative findings of severe fibrosis alongside a cholecystoduodenal fistula.
Received open gallbladder measuring 7 x 2.5 cm. The serosal showed a grey area measuring 2.1 x 0.9 cm, suspicious for a mass or other localized lesion. Sectioning revealed an ulcerated mucosa with grey discoloration. The gallbladder wall measured up to 0.5 cm in maximum thickness.
