Intradural, lobulated, non-enhancing fatty mass at the D11-D12 level that demonstrates fat suppression and compresses the posterior conus medullaris and cauda equina—resulting in proximal cord thinning and myelopathy—with a primary radiological differential of intradural spinal lipoma versus spinal dermoid cyst.
