Necrotic tissue cores show scattered viable foci of malignant tumor formed of acini and trabecular formations of malignant polygonal and focally spindled large cells. Tumor cells have hyperchromatic or vesicular nuclei with prominent nuclei and amphophilic cytoplasm. In view of histomorphology, elevated level of serum AFP and triphasic CT findings, picture is consistent with high grade hepatocellular carcinoma. However, a confirmatory marker study (HepPar1, glypican-3, ERG) is recommended.
