Smears are hypocellular showing few clusters of follicular cells exhibiting rounded and focal enlarged nuclei with modest cytoplasm set in a background rich in colloid material. Some nuclei show clefts. Back ground shows few lymphocytes. Such cytological features are more in consistent with a follicular neoplasm. This classification encompasses adenomas, atypical adenomas, minimally invasive and widely invasive carcinomas. According to Bethesda System for Reporting Thyroid Cytology (BSRTC), this categorized as Bethesda IV with cancer risk about 15-30%. Hence, lobectomy is recommended.
