Skin of nipple shows partial ulceration with extensive intraepidermal infiltration by large cells with clear cytoplasm and vesicular nuclei. These cells are present singly and in small groups. Anaplasia is moderate. No evidence of introduct carcinoma or invasive component in the submitted biopsy.
Picture is consistent with Paget disease of nipple, recommended for confirmatory marker study (SOX10, HER2 and P63).
