Appendicular lumen is distended and appendicular epithelium is extensively ulcerated and reduced in few foci to a flat layer of simple columnar mucigenic epithelium. Foci of ulceration are seen related to dense non-specific inflammation rich in neutrophils, lymphocytes, plasma cells with histiocytes. This infiltrate extends to involve the whole thickness of the thick and fibrotic wall. No evidence of malignancy.

Appendicectomy specimen showing appendix 8.5 cm long with attached mesoappendix measuring 5x1.5 cm. Opening revealed almost complete replacement of appendix by a cyst measuring 8x1.5 cm filled with gelatinous material. Capsule is intact.
