Myometrial tumors are benign leiomyomas formed of whorly bundles of mature smooth muscles separated by a fibrous stroma. No evidence of tumor necrosis. No evidence of atypia or malignancy. Endometrium is non-secretory, it is formed of focally branching glands lined by 1-3 layers of cells. Stroma is cellular. No evidence of complex glandular pattern. Islands of these endometrial glands and stroma are seen entrapped deep within myometrium. Cervix shows napothian cysts and mild ectocervicitis. The Left ovarian cyst is lined by several layers of polygonal luteinized cells with focal interstitial hemorrhage into wall. Both ovaries show corpora albicans and cystic follicles. Both tubes are unremarkable. No evidence of atypia or malignancy.

Panhysterectomy specimen: the uterus measures 8.5x6.5x4 cm with a myometrial thickness of 2.5 cm and an endometrial thickness of 0.4 cm, showing multiple fibroid masses with the largest measuring 2.5x2.5 cm, having a solid, rubbery, whitish cut section. Cervix is 2 cm long. The left ovary is almost replaced by a unilocular cyst measuring 5.5x4.5x2.5 cm. The right ovary measured 3x2x1 cm, grossly unremarkable. Both fallopian tubes measured 5 cm in length, and are grossly unremarkable
