Diagnosis
<p class="ql-align-justify">(1) Gastric mucosa showing mild glandular atrophy and dense infiltration by lymphocytes and plasma cells. Lymphocytes aggregate in some foci to form lymphoid follicles with active germinal centers. Neutrophils are seen infiltrating glands thus indicating activity. H. pylori characteristic forms are seen colonizing glands. No evidence of intestinal metaplasia, atypia or malignancy.</p><p class="ql-align-justify"><br></p><p class="ql-align-justify">(2) & (3) Duodenal and ileal mucosa showing mild infiltration by lymphocytes and plasma cells. Villi are intact and are negative for atrophy and ulceration. No evidence of specific granulomas. No evidence of malignancy.</p>
<p class="ql-align-justify">Abdominal pain and distension. Endoscopy revealed: Severe nodular antral and pangastropathy, Nodularity of terminal ileum.</p>
<p>Three biopsies were received:</p><p class="ql-align-justify">1- Gastric: soft tissue fragments collectively measured 0.9 cm, totally embedded.</p><p class="ql-align-justify">2- Duodenal: soft tissue fragments collectively measured 0.9 cm, totally embedded.</p><p class="ql-align-justify">3- Terminal ileum: soft tissue fragments collectively measured 0.7 cm, totally embedded.</p>
<p><strong><em><u>(1) Stomach, endoscopic biopsy:</u></em></strong></p><ul><li><strong>H. pylori-associated chronic atrophic follicular gastritis with mild inflammatory activity. </strong></li><li><strong>Negative for atypia and malignancy. </strong></li></ul><p><br></p><h3><strong><em><u>(2) & (3) Duodenal & Terminal ileum, endoscopic biopsies:</u></em></strong></h3><ul><li><strong>Mild chronic non-specific inflammation. </strong></li><li><strong>Negative for villous atrophy.</strong></li><li><strong>Negative for malignancy. </strong></li></ul>
