Sections show ovarian stromal tumor composed of intersecting bundles of bland spindle fibroblastic cells within a collagenous stroma. Overall cellularity is mild to moderate. Extensive areas of ischemic (hemorrhagic) infarction are present, characterized by diffuse coagulative necrosis with loss of nuclear detail, eosinophilic ghost outlines of stromal cells, stromal hemorrhage, fibrin deposition, and abundant hemosiderin-laden macrophages. Viable areas at the periphery show preserved bland cytology with smooth nuclear contours, inconspicuous nucleoli, and no cytologic atypia, anaplasia, or increased mitotic activity.
