Sections from the Rt. brownish nodule shows encapsulated tumor formed of small and large follicles full of colloid and lined by cuboidal cells having abundant eosinophilic cytoplasm (oxyphil cells). Capsule looks partially infiltrated by these follicles. However despite extensive sampling and repeated serial sectioning, vascular invasion and unequivocal panmural capsular infiltration could not be documented. Adjacent thyroid tissue is compressed and show focal dense infiltration by lymphocytes amounting at the formation of lymphoid follicles with intact active germinal centers. Related follicles are degenerated with evidence of oxyphil change of the follicular epithelium. No evidence of malignancy or hyperfunction.
Rt. thyroid nodule diagnosed by FNAC as follicular nodule of undetermined significance.
Total thyroidectomy: two lobes and an isthmus with intact lobe. The Rt. Lobe measured 4 x 3 x 1.7 cm; cut sections reveal rubbery brownish capsulated nodule. Lt. Lobe measures 3.5x2x1.5 cm and isthmus measures 1.5 x 1 cm. Thyroid tissue shows multiple gray nodules. Thyroid capsule is intact.
Total thyroidectomy:
