Mural cellular tumor composed of fascicles or whorly bundles of proliferating spindle cells with fusiform or blunt ended (cigar-shaped) elongated nuclei. There is focal palisading of nuclei. Anaplasia is moderate and mitosis is increased (about 7-8/50 HPF). Margins of excision included are positive for tumor infiltration.
Picture is consistent with high risk gastrointestinal stromal tumor (GIST). Marker study (CD117, CD34, actin, DOG1, S100, Ki-67) is recommended to confirm tumor type and to exclude other possibilities of stromal tumors, mainly leiomyosarcoma and malignant peripheral nerve sheath tumor.
