Diagnosis
<ul><li class="ql-align-justify">Partially ulcerated duodenal mucosa showing almost diffuse villous atrophy as the intestinal villi are significantly shrunken or partially flattened (moderate atrophy). The crypts (glands at the base of the villi) become deeper as the body tries to compensate for lost villi. There is a pathological increase in these lymphocytes within the epithelial lining with follicles. No evidence of specific granulomas. No evidence of atypia or malignancy.</li><li class="ql-align-justify">Intact colonic mucosa mildly infiltrated by lymphocytes and plasma cells. Mucigenic cells are intact. Some neutrophils are seen infiltrating glands but there is no evidence of crypt abscesses. No evidence of atypia or malignancy.</li></ul>
<p class="ql-align-justify">Diffuse duodenal scalloping and fissuring with mild congested mucosa at right colon.</p>
<p>Two biopsies were received:</p><p class="ql-align-justify">1- Duodenal: soft tissue fragments collectively measured 0.6 cm, totally embedded.</p><p class="ql-align-justify">2- Colonic: soft tissue fragments collectively measured 0.6 cm, totally embedded.</p>
<p><strong><em><u>Duodenum, endoscopic biopsy: </u></em></strong></p><ul><li class="ql-align-justify"><strong>Moderate villous atrophy, consistent with Marsh Class III (3B) according to Marsh-Oberhuber classification of celiac disease. </strong></li><li class="ql-align-justify"><strong>Associated moderate chronic follicular non-specific inflammation. </strong></li><li class="ql-align-justify"><strong>Negative for atypia or malignancy. </strong></li></ul><p class="ql-align-justify"><br></p><p><strong><em><u>Colon, endoscopic biopsy: </u></em></strong></p><ul><li><strong>Mild chronic non specific colitis with minimal inflammatory activity. </strong></li><li><strong>Negative for dysplasia and malignancy. </strong></li></ul><p class="ql-align-justify"><br></p>
