1- Gastric mucosa showing mild atrophy of glands and moderate infiltration by lymphocytes and plasma cells. There is evidence of inflammatory activity manifested by neutrophils infiltrating glands, however H. pylori characteristic forms are not seen. No evidence of intestinal metaplasia, atypia or malignancy.
2- Polypoid mucosal lesion composed of branching, back-to-back glands and slender villous fronds (mixed tubular and villous architecture). Epithelial lining shows low-grade dysplastic changes: mild-to-moderate nuclear enlargement with elongation, hyperchromasia and pseudostratification confined to the basal half of the cells, with preserved polarity and only occasional mitoses. Surface maturation is reduced but present; no marked pleomorphism, necrosis, complex cribriforming, or stromal/lamina propria invasion identified. Stroma shows mild chronic inflammatory infiltrate.

2- Cardiac lesion: soft tissue fragments collectively measured 0.7 cm, totally embedded.
