Sections from both sides show caseating granulomas rich in epithelioid cells, lymphocytes, plasma cells and Langhan's giant cells. Some granulomas show evidence of secondary infection manifested by central neutrophilic accumulation. Inflammation extending to surrounding tunica with related mild fibrosis. Adjacent epididymal tubules are empty. No evidence of atypia or malignancy. Picture is consistent with a tuberculous etiology. Confirmatory microbiological studies and skin testing are needed for confirmation.
Rt and Lt spermatic cord masses.
Two soft tissue masses were received:
Rt: measured 1.5x1 cm, totally embedded.
Lt: measured 3.3x1.5x1 cm, totally embedded.
