The small nodule shows benign tumor composed of small follicular formations lined by active follicular cells with a small amount of colloid in the majority of follicles. Separating stroma is delicate and vascular. No evidence of vascular invasion or panmural capsular infiltration.
Rest of thyroid tissue shows nodular hyperplasia where follicles are variable in size, full of thick colloid and lined by mature epithelium. There is focal follicular cystic change observed in the left lobe sections. Stroma shows fibrosis, interstitial hemorrhage and cholesterol crystals deposition. No evidence of hyperfunction or malignancy.

Total thyroidectomy specimen consisted of two thyroid lobes and a 1 cm isthmus with intact capsule. The first lobe measured 5.3 x 3.5 x 3 cm, was almost replaced by gray and cystic 4.5 cm nodule. The second lobe measured 4 x 1.5 x 1 cm, contained a single, whitish 0.8 x 0.7 cm nodule.
