Dilated pelvicalyceal system is lined by urothelial lining with extensive ulceration and underlying moderate to dense chronic inflammatory cell infiltrate dominated by plasma cells, histiocytes, lymphocytes and many neutrophils amounting at abscess formation in many areas. Lymphocytes aggregate in many foci forming lymphoid follicles with active germinal center. Sections of residual kidney tissue show marked fibrosis, glomerular & tubular atrophy with hyaline casts. Inflammation extends to the perinephric fat. No evidence of specific granulomas. No evidence of atypia or malignancy.

The specimen consists of a kidney measuring 14 × 8 × 7 cm with an attached ureter ice stump measuring 3 cm in length and surrounded by perinephric adipose tissue measuring 5 × 2.5 cm. On serial sectioning, the renal parenchyma shows multiple cystic spaces, the largest measuring 2 × 2 cm, some of which are filled with purulent material. The pelvicalyceal system contains multiple brown-colored calculi, the largest measuring 1.5 cm in greatest dimension. No discrete solid mass is identified on gross examination.
