Sections show fragments of extensively ulcerated esophageal mucosa. The ulcer base is composed of fibrinopurulent debris overlying granulation tissue with a dense mixed acute and chronic inflammatory cell infiltrate. The adjacent squamous epithelium shows reactive regenerative changes without dysplasia. No viable neoplastic cells are identified in the examined material.

Comment:
The endoscopic finding of a large fungating mass at the gastroesophageal junction is not explained by the current biopsy, which demonstrates only ulcerated inflamed tissue. Correlation with the endoscopic and radiologic findings is strongly recommended. Repeat biopsy with multiple deep tissue samples from the mass lesion is advised to exclude an underlying unsampled malignancy, as the current specimen may not be representative of the lesion.
