Sections show polypoid gastric mucosal fragments with a tubulovillous architecture, composed of crowded glands and villiform projections lined by intestinal-type epithelium with goblet cells. Most areas show low-grade dysplasia with nuclear elongation, hyperchromasia, and pseudostratification. Focal high-grade dysplasia is present, characterized by marked nuclear enlargement and pleomorphism, loss of polarity, increased nuclear-to-cytoplasmic ratio, and increased mitotic activity. No evidence of frank malignancy.
Rest of gastric mucosa shows severe chronic gastritis with moderate activity with intestinal metaplasia. Helicobacter pylori is not identified.
