Rt and Lt thyroid lobes show nodular adenomatous hyperplasia. Nodules are variable in size and composed of small and large crowded follicles full of thick colloid and lined by mature epithelium. Sections from some of the nodules show compact microfollicles devoid of colloid and lined by cuboidal cells separated by delicate vascular stroma. In these follicles nuclei are vesicular and look active but no appreciable mitosis. Stroma shows mild fibrosis. No evidence of hyperfunction or malignancy.
Mutinofulocysruc goiter grade III with severe hyoervascularity.
Total thyroidectomy specimen showing Rt lobe measuring 5x1.5x1.3 cm, Lt lobe measuring 4.5x3x2.5 cm and isthmus measured 1.5x1 cm. Serial sectioning revealed Rt lobe multiple nodules ranging in size from 0.6 cm to 1 cm and 3 nodules in the Lt lobe with the largest measuring 4x2.7x2.3 cm.
