Sections from grossly detect nodule show an infiltrative malignant epithelial neoplasm composed of solid nests, trabeculae, and sheets of polygonal cells separated by fibrous stroma. Tumor cells exhibit moderate eosinophilic cytoplasm, distinct cell borders, and vesicular nuclei with conspicuous nucleoli. Focal lymphoplasmacytic infiltrates are present within the tumor stroma. No papillary architecture, psammoma bodies, keratin pearl formation, or intracytoplasmic mucin is identified. Definite thyroid follicular differentiation is absent. Tumor infiltrating thyroid capsule and extending to surrounding strap muscle. Picture of undifferentiated carcinoma. Immunohistochemical studies are recommended for diagnostic confirmation (CD5, P63, CK5/6, CD117, TTF-1, Thyroglobulin, Synaptophysin, HBME-1, CK19, Calcitonin & KI-67)
Total thyroidectomy.
Total thyroidectomy specimen was received as two thyroid lobes, showing first lobe measuring 4.5x2x1.2 cm, second lobe measuring 3x1.7x1.2 cm. Serial sectioning revealed a rubbery nodule in the larger lobe measured 1.5x1 cm, grossly infiltrating capsule. Cut section of the second lobe was fleshy.
