Sections reveal fragments of gastric mucosa with some of them showing lamina propria extensively infiltrated by a diffuse proliferation of discohesive atypical cells, exhibiting prominent, large intracytoplasmic vacuoles that displace and compress the nuclei against the cell membrane (signet-ring morphology). These cells display nuclear hyperchromasia and pleomorphism. No definitive gland formation or extracellular mucin pools are identified. Background shows H pylori associated moderate chronic gastritis with moderate inflammatory activity and focal intestinal metaplasia.
Picture is highly suggestive of poorly cohesive carcinoma, signet ring cell type, recommended for confirmatory immunohistochemical studies (CK, CD20, CD3, CD68).
