1- Ileal and colonic mucosa showing mild infiltration by lymphocytes and plasma cells. Ileal villi are intact and are negative for atrophy and ulceration. No evidence of crypt abscesses. No evidence of specific granulomas. No evidence of malignancy.
2- Polypoid lesion formed of hyperplastic intact colonic glands with preserved mucigenic activity. Stroma is fibrotic and shows moderate chronic inflammatory reaction composed of lymphocytes, neutrophils and plasma cells. No evidence of specific granulomas. No evidence of crypt abscesses. No evidence of atypia or malignancy.
Chronic diarrhea.
Two biopsies were received:
1- ileum and ileo-cecal valve biopsies: soft tissue fragments collectively measured 1 cm, totally embedded.
2- Single polyp at junction of ascending colon and cecum: soft tissue fragments collectively measured 1 cm, totally embedded.
